First Time Visitors

We will schedule your appointment with Dr. Elbaz to ensure you have his undivided attention, giving you the time to ask questions and ensure we meet your individual needs. Please assist us at the time of your initial visit to the office by bringing the following information with you:

  • Referral slip and x-rays from your referring dentist

  • Completed patient registration form – you may download the form below

  • List of medications you are currently taking (unless already stated in your patient registration form)

  • Dental insurance forms or insurance cards (if you have dental insurance)

  • All patients under 18 must be accompanied by a parent or guardian at the consultation appointment

Insurance Coverage

Our insurance coordinators deal with different insurance companies. Some companies offer many different dental and medical plans. These companies can change benefits, co-pays, and deductibles many times throughout the year. Most dental insurance policies are limited and often only pay for a portion of the procedure(s) that may need to be done.

CO-PAYS

We do our best to provide you with accurate coverage estimates based on information available to us. At times, it is almost impossible to accurately estimate a patient’s insurance co-payment. Many insurance companies will not give out the fees they base the patient’s benefits on to the dental provider and/or until after the treatment is completed. Dealing with these companies can be difficult and time consuming.

CLAIMS

As a courtesy, we ask that you keep us informed of any change to your insurance. It is important that all information about you and your insurance is current. Although we will gladly file a claim on your behalf, you may wish to submit the claim yourself. In general, insurers process claims filed directly by patients faster than those filed by the service providers (dental offices).

Financial Policy

We ask all of our patients to take care of their fees on, or before, the day of their treatment. For your convenience, we accept cash, checks, money orders and credit card payments (MasterCard, Visa, American Express and Discover). If you wish to make extended payments or participate in a payment plan, we participate with Care Credit. Care Credit is a dental credit card company that may extend credit for your treatment needs with a lower interest rate than most other credit card companies offer.

INSURANCE

We are also a provider for Delta Dental Insurance and ask those patients who have Delta Dental Insurance to pay the estimated portion of their treatment on, or before, the day of their treatment. For those who may have any other dental insurance, we ask that you take care of your fees on, or before, the day of your treatment. We are happy to process your dental claims and will ask your dental insurance company to send all reimbursement checks directly to you.

CANCELLATIONS

We strive to provide the utmost in professional service to our patients. Appointment times are reserved exclusively for our patients and we request 48 hours notice of cancellation in order to avoid any broken appointment charges.

Frequently Asked Questions

Dr. Elbaz will complete an in-depth medical/dental health history and a thorough examination to measure for bone loss, loose teeth, bite, oral cancer screening, TMJ, and other signs of periodontal disease. He will also review any necessary diagnostic x-rays, as well as information provided by your dentist, so that he may thoroughly evaluate your clinical situation and determine the proper treatment plan to meet your needs. Following your examination, Dr. Elbaz will discuss several treatment options, their respective benefits and drawbacks, and explain the procedures involved.

We will be as gentle as possible. The periodontal exam can be completed with little or no discomfort.

We will need current x-rays in order to see disease not otherwise visible. If your referring dentist has taken x-rays, you may request that they be forwarded to us.

Since all patients are different, Dr. Elbaz must complete your examination before establishing your treatment plan and the fee for such care. The fee for periodontal treatment can vary considerably depending on the type of problems and the complexity and length of treatment. An estimated fee can usually be determined at the initial visit.  On occasion, further diagnostics must be completed before the final treatment plan and cost can be determined.

Dental insurance policies often cover periodontal treatment, but there is no guarantee of coverage. Please bring all medical and dental benefit information and cards to your examination appointment. Upon request, we will submit a claim to predetermine your insurance benefits; however, this is not required by most plans.

Patients must acknowledge that they are responsible for all costs incurred at our office which are not covered by their insurance.

Not everyone needs periodontal surgery. If treated early, gum disease can be controlled without surgery. We will make recommendations based on your individual situation. Our philosophy of practice is to treat as conservatively as possible to attain treatment goals.

The recent advances in periodontal treatment allow us to successfully treat most teeth.

Our office and your dentist will work closely together. If crowns and fillings are needed, your dentist will provide them. Regular visits to your dentist are an important part of maintaining your dental health.

Periodontal disease is a progressive, painless infection. Delay can cause you further bone loss, more expense, and the loss of your teeth. If your teeth are lost, dentures are never as effective as your own natural teeth.
