We understand your time is valuable and your scheduled appointment with Dr. Elbaz is to ensure you have his undivided attention to address all your individual needs. We wish to provide acomfortable platform for open communication with our patients, so that you may ask all questions necessary in order to help us deliver more personalized, comprehensive care to address your needs.
Please assist us at the time of your initial visit to the office by bringing the following information with you:
- Photo Identification
- Dental Insurance Forms or Insurance Cards (if applicable)
- Completed Patient Registration Forms
- List of Medications (Current medications you taking unless already stated in your Patient Registration Forms)

Our office is located on the 4th Floor (Suite #450) of the Chase Bank building, located on the North-West corner of Beverly Drive & Wilshire Boulevard, in the heart of Beverly Hills. There are several parking structures conveniently located nearby offering one or two hours of free parking.
